If you are interested in joining the SMFC NLYM chapter, please complete the form below. Please note that all Prospective Mother Members participate in the Membership Selection Process and must be sponsored by two Active Members of the NLYM Chapter. Membership is subject to space availability.

Only a Prospective Mother should complete this form (not their sons). SMFC Chapter members must reside or attend school in one of the following cities: San Mateo, Foster City, Hillsborough, Burlingame, or Belmont.
